Forms Database

Forms Database is an online issue tracking service that allows you to distribute forms on the internet, assign them to staff within your organization, and view detailed reports of their activity. Forms Database makes managing forms simple and provides customers quick access to information without the need to contact your support staff directly. By establishing an automated solution to managing customer feedback forms, organizations can take advantage of storing submitted data in a single repository. This makes retrieving and reporting form activity simple and efficient. With Forms Database, there is no limit to the variety of forms you can create.

Comprehensive Form Management

For example, City Management needs to implement a comprehensive system to manage citizen code enforcement complaints. Forms Database allows them to develop a custom form for each type of code enforcement violation and then route the submissions throughout the organization. Another business might want to develop a customer service center, while another focuses on pay per click advertising leads. These are just some examples of how Forms Database can be used. The common thread between all of these is Forms Database’s ability to create custom forms, save the submissions in a database, and route the data to the appropriate party.

Form Design

Creating the form is simple and can be done in any HTML environment. As you build the form, the input fields are strategically named as they will eventually be used to dynamically generate a database to store the form submissions.

Add a Form and Set Assignees

Next, you will upload the form in Forms Database along with a return page which will display after the form is submitted. After designating the form and return page, you then have the option to choose who will receive the form data upon submission. The form data can be routed via one of two ways: you can either create an assignment that must be closed out and can be reported on or you can set the results to be carbon copied to someone who wants to be aware of all activity on the form.

Automatic Database Creation

Quite possibly, the most powerful features of Forms Database is its ability to dynamically generate SQL tables based on a designed HTML form. As you design the form and upload it into the Solodev system, a table is created.

Assignments Overview

As forms are assigned throughout your organization, they will appear in the assignments overview screen. In Forms Database, this screen is available when you first log in making management of form submissions a top priority. Simply click on the assignment ID to be handled and you will be forwarded to the assignment detail screen.

Assignments Details

Assignments display the time of submission, author, tracking number, current user assignments, and the assignment status.

You have the option to update the original submission, add attachments, respond to the submission, or assign it another member of your organization.

Reports Generation

Forms Database has been designed with department managers in mind. While staff members keep up with day-to-day submissions, the report options allow management to create performance dashboards that indicate the timeframes for which customer requests are being handled. You will clearly be able to see who is handling submissions and the efficiency in which they are being processed.
